Job Summary Hybrid Schedule – 40 hour per week, three days remotely and four days onsite. This role supports Internal Medicine and Family Medicine teams, providing social work assessment, care coordination, crisis intervention, and advocacy in ambulatory settings. Job Opportunity We are seeking a full-time Social Worker (Clinical Social Worker or similar) to join our Internal Medicine and Family Medicine teams. The position requires a Master’s degree in Mental Health & Behavioral Medicine and 2–3 years of clinical experience in a medical setting. Key Responsibilities Perform psychosocial assessments of patients and families addressing social, emotional, interpersonal, or environmental issues. Formulate biopsychosocial assessments, dispositions, and treatment plans. Collaborate with other professionals to evaluate patients' medical or physical conditions and assess client needs as part of an interdisciplinary team. Monitor, evaluate, and record client progress according to measurable goals. Essential Functions Provide psychosocial assessments and interventions for inpatients, outpatients, and ED patients. Develop and implement comprehensive treatment plans that integrate medical, social, and resource needs. Work collaboratively with the healthcare team to support patient-centered care. Qualifications Education: Master’s Degree in Mental Health & Behavioral Medicine (required). Experience in lieu of a degree is not accepted. Licenses & Credentials: Class D Passenger Vehicle Driver's License – preferred. Licensed Clinical Social Worker (LCSW) – preferred. Licensed Independent Clinical Social Worker (LICSW) – preferred. Licensed Mental Health Counselor (LMHC) – preferred. Experience: 2–3 years of clinical experience in a medical setting. Knowledge, Skills & Abilities Proficient clinical judgment and application of professional values and ethics. Assessment and documentation of risks, crisis interventions, and treatment plans. Effective advocacy while maintaining professional boundaries. Knowledge of internal and external resources and appropriate referrals. Evidence-based interventions. Advocates, coordinates, and communicates treatment plans with patients, families, and interdisciplinary team. Incorporates social sensitivity and utilizes supervision and consultation appropriately. Seeks assistance for complex cases timely. Engages in quality improvement projects and uses data to measure progress. Facilitates skill-based groups. Negotiation skills and ability to work in a complex organization. Effectively communicates findings to a broader audience.
