Therapist - Speech Language Pathologist

Job Description

Job Summary:

Provide specialized speech-language pathology services to patients with communication, cognitive, or swallowing disorders. Develop and implement individualized treatment plans to enhance patient outcomes within regulatory standards.

Responsibilities:

• Assess patients' communication, cognitive, and swallowing functions using standardized evaluations.
• Design and execute evidence-based treatment plans tailored to individual needs.
• Monitor and document patient progress, adjusting interventions as necessary.
• Collaborate with healthcare team members, patients, families, and caregivers to ensure comprehensive care.
• Conduct discharge planning and prepare detailed service summaries in compliance with clinical protocols.
• Maintain accurate documentation of services, treatment minutes, and regulatory compliance in electronic records.

Qualifications:

• Hold a valid Texas State License as a Speech-Language Pathologist.
• Possess active ASHA certification and CCC credential.
• Have a minimum of 1 year of relevant Speech-Language Pathology experience; LTC or rehab experience preferred.
• Demonstrate ability to supervise CFY or student SLPs.
• Be compliant with ASHA and state practice act, code of conduct, and ethical standards.
• Completion of required licensure submissions and active state licensure.
