Summary

Full-Time. Monday - Friday. 8am - 4:30pm.

Additional Information

Provides and promotes a proficient level of nursing practice to efficiently provide safe, effective, and holistic patient and family centered care for the patients and families with complex needs. Uses increased knowledge, skills, and experience to understand situations holistically and plans and adjusts accordingly. Quickly recognizes variations from the anticipated course, thinks critically, modifies plans, makes appropriate decisions, recognizes salient patient care issues, and focuses on long term goals.

Required Education

Associate's degree or equivalent from two-year college program or technical school or nursing diploma

Demonstrated performance of 100% at the previous level and score within documented RN II/III levels via ACHieve career ladder for internal employees., Nursing - 3 years of experience

1. Demonstrates and promotes excellence in the application of the AC Model of Patient and Family-Centered care. Collaborates and role models partnerships with patients, families and other providers to plan, deliver, and evaluate highly individualized care that ensures achievement of effective self-care by patients and families. Consistently plans and delivers high quality, comprehensive and holistic care for complex patient situations. Assesses completeness of all patient and family education and recognizes additional needs in comprehensive interdisciplinary teaching.6. Advances evidence-based nursing practice using best available evidence, patient's and family's preferences and values, and team's clinical expertise at a proficient level.
