What You'll Do:

• To provide support to staff and provide telephone interventions to maintain patients safely in their home, including scheduling home visits as needed.
• Collaborate with Physician practices and hospital staff to influence effective management of patients in their homes.
• Assess and identify the patient's holistic health care needs.
• Demonstrate competency in assessment and technical skills.
• Possess and utilize knowledge of pain control & symptom management.

What You'll Bring:

• Current R.N. license in the State of Maine.
• ANA certification in Home Health or Community Health Nursing is encouraged.
• Current CPR card required.
• One year of recent clinical nursing experience is required.
• A reliable car with insurance.
