Health Care Coordinator

Position Summary Conduct assessments during the participant’s intake phase and as needed to understand Veterans’ health care status, potential barriers to care, and the impact of such barriers on their ability to access and maintain health care services. Responsibilities Assist veterans in communicating health care preferences and personal health goals to facilitate shared decision‑making. Assist veterans in preparation for medical appointments, ensuring all issues are addressed and veterans are informed about health care plans. Connect veterans not already enrolled or engaged in health care to VA health care or other medical care providers, and provide education on wellness‑related topics and healthcare plan options. Collaborate with the Veterans multi‑disciplinary team (medical, mental health, case and housing coordination personnel) to identify and address challenges and develop solutions. Act as a health coach by proactively supporting the Veteran to access and appropriately utilize treatment interventions and outcomes. Arrange transportation to medical appointments and remain current on community medical resources available. Transport Veterans to appointments, housing searches, or other necessary locations, ensuring safety and timeliness during travel. Attend weekly Veteran Case Conferencing meetings to discuss Veterans with high‑level mental and physical health needs. Maintain documentation of trainings, client contacts, and other pertinent information in accordance with agency and best standard practices. Requirements Age 18 years or older. At least 2 years of professional experience in a social service role; experience in a medical/hospital environment conducting medical social work duties preferred; prior experience with homeless populations a plus. Successful completion of pre‑employment drug screening and background check. Minimum of a Bachelor’s Degree in Social Work; LCSW preferred; ASW acceptable; Master’s Degree preferred. Proficient computer literacy (MS Office) and ability to maintain well‑organized records and files. Ability to travel within and throughout the counties in which services are provided at least 25% of the time and within the state on an as‑needed basis. Physical demands: ability to stand, walk, sit, talk and hear; lift up to 25 lbs; spend majority time seated; repetitive finger, hand, wrist motion. Proficient skill in Microsoft Office (Word, Excel, PowerPoint, Zoom, Teams, SharePoint). Well‑developed interpersonal skills; strong analytical and problem‑solving skills. Benefits Full‑time position with a salary range of $25,000–$28,000. The role is part of a non‑profit/social services organization dedicated to veterans’ care.
