Job Details Facilities: Pivot at Park Valley Inn Health Center - Round Rock, TX 78681 Position Type: Full Time Job Category: Clinical Support Location: Park Valley & Falcon Ridge OR Bel-Air & San Gabriel Rehabilitation Responsibilities Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care. Perform restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training. Promote and support the greatest possible degree of independence for patients. Advises Director of Rehab (DOR) of patient's status daily. Contribute knowledge of patient's conditions/needs to patient care plans. Assist in the implementation of care plans as directed within the parameters of established policy and procedure. Observe and report on patient's conditions and document observations as required. Qualifications State certification as a Nursing Assistant is mandatory. Ability to read, write, and speak the English language. Must be available to work six days per week. Ability to travel to additional SNF locations within geographical area.
