SOUTH Brooklyn Hospice RN Case Manager – 15K New Hire Bonus, PENSION and 4‑day Work Week Salary: $101,000 - $116,800 per year. Location: Leading Hospital, Brooklyn (onsite). Full‑time role offering a 4‑day work week with 10‑hour shifts. Must work two full weekends per month (Saturday and Sunday). The Planner facilitates admission of referrals upon hospital discharge to a Hospice program that meets the needs of the patient and family. As a key member of the business development and marketing team, the Planner develops and maintains positive relationships with physicians, social workers, case managers and discharge planners within the medical center. The role also involves: Serving as a key intermediary between hospital staff, patients, and families to ensure clear communication and coordination of hospice care. Conducting thorough chart assessments and bedside evaluations to determine hospice eligibility. Collaborating with facility clinical team and hospice physicians to make informed decisions regarding patient care. Successfully facilitating hospice admissions ensuring seamless transitions for patients and families. Providing critical support and education to enhance patient and family understanding of hospice care, leading to improved satisfaction and care outcomes. Overseeing the coordination of admission assessments and procurement of durable medical equipment, ensuring all patient needs are met promptly.
