Clinical - Care Manager (NICU & OB RN)Immediate need for a talented Clinical - Care Manager (NICU & OB RN). This is a 06+months contract opportunity with long-term potential and is located in U.S(Remote). Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).Key Responsibilities:Review referrals and risk stratification reports to identify care needs.Conduct comprehensive trimester-based assessments and develop individualized care plans.Coordinate services with providers and community partners, ensuring seamless care transitions.Engage in member outreach to address gaps in care and document interventions in the care management platform.Collaborate with interdisciplinary teams to support high-risk pregnancy management and postpartum engagement initiatives.Monitor compliance with regulatory standards and track performance metrics related to quality and member engagementKey Requirements and Technology Experience:3+ years of recent RN clinical experience.5+ years of nursing experience in care management, maternal-child health, OB, NICU, pediatrics, population health, Medicaid, managed care, or community health.Active, unrestricted RN license required.Strong critical thinking and clinical assessment skills.Exceptional communication and motivational interviewing abilities.Highly organized and self-directed with a passion for improving maternal and infant health outcomes.Experience in case management.Our client is a leading Healthcare Industry, and we are currently interviewing to fill this and other similar contract positions.
