- Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular conditions.
- Develop individualized treatment plans using evidence-based clinical practice.
- Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
- Prescribe medications and treatments within your scope of practice.
- Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
- Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
- Maintain accurate, timely, and compliant clinical documentation.
- Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
- Build strong relationships with facility leadership and clinical teams.
- Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.
- Current, unrestricted Nurse Practitioner in the applicable state(s) of practice.
- Current national certification appropriate to licensure.
- Current Basic Life Support (BLS) certification.
- Minimum six (6) months of Nurse Practitioner clinical experience.
- Valid driver's license with reliable transportation.
- Ability to travel daily throughout an assigned territory.
- Strong clinical judgment, communication, organizational, and critical thinking skills.
- Experience utilizing Electronic Medical Records (EMR).
- Two (2) or more years of Nurse Practitioner experience.
- Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
- Experience caring for medically complex patient populations.
- Practicing specialty medicine with meaningful clinical autonomy.
- Collaboration with experienced physicians and supportive clinical leadership.
- Established relationships with Skilled Nursing and Long-Term Care partners.
