Family Nurse Practitioner
Elevance Health
Elevance Health seeks a Family Nurse Practitioner to provide comprehensive primary and preventive care to diverse member populations. In this role, you'll perform advanced assessments, diagnose and manage acute and chronic conditions, order and interpret tests, prescribe medications, and develop whole-person care plans. You'll deliver care in a hybrid model (in-person and virtual), collaborate closely with multidisciplinary teams, and use EHR and telehealth tools to coordinate services. Ideal candidates bring strong clinical judgment, a commitment to health equity, and passion for community impact and continuous learning.
