Social Worker LCSW Location: Newark Beth Israel Medical Center, 201 Lyons Avenue, Newark, NJ 07112. Department: Advanced Heart Program. Position: Full‑time, Day shift, 35 hours per week. Salary range $78,877.50 – $96,720.00 per year. Job Overview RWJBarnabas Health is seeking a Clinical Social Worker to join the Advanced Heart Department. This position provides direct patient‑centered care and advocacy within an interdisciplinary healthcare team. Responsibilities include conducting psychosocial assessments, providing counseling, managing discharge plans, connecting families with community resources, and ensuring seamless post‑acute continuity of care. Qualifications Licensed Social Worker with a Master’s Degree in Social Work from an accredited school. Experience working in a health care setting or human services organization. LCSW preferred. Two or more years experience as a transplant, renal or cardiac social worker highly preferred. Scheduling Requirements Full time – 35 hours per week. Day shift. Essential Functions Conduct comprehensive psychosocial assessments and determine patient’s capacity to receive an organ and/or VAD. Document all evaluations and interactions in the patient record in a timely manner. Communicate findings to the multidisciplinary team. Facilitate therapeutic support groups or educational sessions for patients and families. Identify and employ interventions to reduce psychosocial risk factors and improve transplant/VAD outcomes. Act as a professional social worker throughout all phases of transplant. Advocate for fair and equitable access to services. Demonstrate knowledge of community resources and funding opportunities. Collaborate with multidisciplinary transplant teams while adhering to professional roles and boundaries. Participate in departmental patient review activities (e.g., patient selection, daily rounds, financial review meetings, patient education). Work with Financial coordinator to identify avenues to assist patients with identified needs. Maintain access to assistance for transplant specific and organizational resources. Collaborate with hospital social work and case management to assist in identifying patient needs and resolution. Other Duties Additional duties may be assigned by management and may change over time.
