Hospice RN Case Manager

As a Hospice RN Case Manager with InterimHealthCare of Oklahoma City, you'll manage your own caseload, build real relationships, and deliver the kind of one-on-one care that hospice is meant to be. Let's talk schedule Our hospice census supports a part‑time case manager caseload, and we want to be upfront about that. Here's how it works: If part‑time hospice works for your life, we can keep you hospice only. If you want full‑time hours, we can get you there by supplementing your hospice caseload with home health visits. Either way, we'll build something that works for you. What you'll earn Competitive salary, whether you're hospice only or supplementing with home health for full‑time hours Paid mileage at $0.64/mile Weekly pay every Friday What you'll enjoy (full‑time team members) Health, dental, and vision coverage 401(k) with company match Paid time off and paid holidays $25,000 company‑paid life insurance policy Stay Pay: a $100 bonus for each year of service Employee recognition program with monthly drawings What you'll do Manage a hospice caseload, coordinating care for patients and their families Complete assessments, develop and update plans of care, and document visits Manage symptoms and comfort, and educate families on what to expect Work alongside the hospice team, physicians, aides, social workers, and chaplains Be a steady, compassionate presence for patients and families at end of life What we're looking for Active RN license in Oklahoma Hospice experience required Knowledge of hospice regulations and the Medicare hospice benefit Strong clinical judgment, communication skills, and a heart for end‑of‑life care Reliable transportation and a valid driver's license If this is the work you were made for, we'd love to meet you.
