Job Title: Registered Nurse (RN) Case Manager Job Location: Downtown Kansas City, MO to Northland Company: Specialized Home Care Job Summary: We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on‑one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction.
