Key Responsibilities\n

• Provide emotional support and guidance to patients and their families who are experiencing or have recently faced behavioral health crises.

• Handle case referrals by assessing patients' resource needs and identifying care gaps, while continuously monitoring case progress and implementing appropriate closure strategies.

• Facilitate recovery goals for patients and their families through psychoeducation and effective linkage to community resources and support networks.

• Evaluate social determinants of health that may impede access to care, such as housing insecurity, financial barriers, and transportation challenges.

• Connect patients and families to community groups and mental wellness aids to foster a supportive environment.

• Collaborate with an interdisciplinary team to develop and implement individualized treatment plans that align with patient and family goals.

• Participate in regular interdisciplinary consultations to review patient progress and adapt care plans as needed.

Qualifications\n
