Social Worker
Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team at Buckeye Hospice and Palliative Care Services in Greenfield, Ohio. As a Social Worker, you will work closely with patients and their families to provide emotional, social, and financial support during a challenging time in their lives. Responsibilities:
Provide Medical Social Services to patients and their families according to the Plan of Care
Conduct psychosocial assessments and develop interventions to address emotional, social, and financial needs
Participate in Interdisciplinary Group (IDG) meetings and develop patient care plans
Provide emotional support and counseling to patients and their families
Identify and address gaps in care and make referrals to community resources
Maintain accurate and thorough records of patient care and interactions
Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
Master of Social Work (MSW) degree from a Council on Social Work Education (CSWE) accredited program OR Bachelor of Social Work (BSW) degree from a CSWE accredited program with 1 year of social work experience in a healthcare setting
Licensed to practice social work in the State of Ohio (LSW, LCSW, or LISW)
1 year of social work experience preferred
Hospice experience preferred
Ability to communicate effectively with patients, families, and healthcare professionals
Ability to work independently and as part of a team
Ability to maintain confidentiality and respect patient boundaries
Ability to travel to patient homes and other locations as needed
