Job Overview As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will play a vital role in helping patients regain strength, mobility and independence from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations and enjoy a better quality of life. Responsibilities Evaluate, direct and provide speech/language pathology service to patients in the home or facility. Participate in the development and periodic review of the Plan of Treatment and Plan of Care. Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions. Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening. Observe, record and report changes in the patient’s condition and response to treatment to supervisor and/or the physician. Provide instruction and training to patients in use of alternative communication systems when appropriate. Provide counsel and instruction to patients, families and healthcare staff. Maintain and submit documentation as required by the Company and/or facility. Prepare and submit written reports of evaluations, visits, summaries, care plans, care coordination activities and progress reports as required by Company policy. Participate in care coordination activities and discharge planning. Maintain the highest standards of professional conduct concerning confidential information. Qualifications Required: Minimum six months experience as a speech therapist / speech language pathologist. Current and unrestricted license. Current CPR certification. A valid driver’s license, auto insurance, and reliable transportation. Preferred: Home Health experience. Job Requirements Role is patient facing and is part of the TB screening program; TB screening is required. Role is part of the driver safety program; must have a valid state driver’s license and maintain personal vehicle liability insurance per state minimum limits. Pay and Working Hours Pay Per Visit/Unit Rate: $54.00 – $76.00 Scheduled Weekly Hours: 1 Full‑time (40 hours per week) pay range: $85,400 – $117,500 per year.
