Counselor

Position Title: Counselor (LSW, LPC, or LCDC II/III) Department: Clinical Reports To: Site Director FLSA Status: Exempt, Learned Professional Employment Type: Full-Time, Salaried Talbot Health Services is a growing behavioral health and addiction treatment provider serving Northwest Ohio. We are seeking a licensed Counselor to join our Toledo clinical team. This role is ideal for a mission-driven professional who values person-centered care, strong documentation standards, and ethical clinical practice. This position delivers mental health and substance use disorder treatment services in a trauma-informed, recovery-oriented environment. Services are provided in alignment with CARF Behavioral Health standards, OhioMHAS regulations under OAC 5122-26, Medicaid documentation requirements, HIPAA, and 42 CFR Part 2 confidentiality standards Overview The Counselor is responsible for conducting clinical assessments, developing individualized treatment plans, providing evidence-based counseling services, and maintaining accurate, audit-ready documentation. The role requires sound clinical judgment, professional integrity, and compliance with all applicable licensure board standards. Key Responsibilities Conduct screenings, biopsychosocial assessments, diagnostic summaries within credential scope, and ASAM-informed treatment planning Provide individual, group, and family counseling using evidence-based practices Deliver services consistent with trauma-informed and culturally responsive care principles Complete timely, accurate clinical documentation in accordance with Medicaid and agency standards Coordinate care with medical providers, case managers, and community partners Provide crisis intervention and safety planning when clinically indicated Maintain productivity standards while ensuring quality and compliance Participate in supervision, required in-service training, and quality improvement initiatives Protect all protected health information in compliance with HIPAA and 42 CFR Part 2 Minimum Qualifications Active Ohio LSW, LPC, or LCDC II/III license in good standing Bachelor’s degree in behavioral health or related human services field Background eligibility consistent with OhioMHAS provider requirements and Medicaid credentialing standards Preferred Qualifications LISW, LPCC, or LICDC license Master’s degree in social work, counseling, psychology, or related field Experience in addiction treatment or community mental health CPR/First Aid/AED certification Crisis prevention and de-escalation training Knowledge, Skills, and Abilities Strong understanding of behavioral health and substance use treatment models Ability to apply trauma-informed and recovery-based approaches Effective communication and documentation skills Strong organizational ability and independent decision-making Knowledge of community resources and referral systems Ability to maintain professional boundaries and ethical standards Smoke-free and drug-free workplace Routine interaction with individuals experiencing mental health and substance use challenges Occasional local travel may be required Ability to lift up to 15 pounds and remain seated for documentation tasks as needed Why Join Talbot Health Services Talbot Health Services is committed to providing high-quality behavioral healthcare while supporting our clinicians with strong leadership, regulatory integrity, and a collaborative team environment.
