About the Role

As a Licensed Practical Nurse with Advocate Healthcare of East Boston, you will be responsible for providing direct patient care to our residents, including administering medications, performing vital signs, and assisting with daily living activities. You will also be an integral part of our interdisciplinary care team, working collaboratively with other healthcare professionals to develop and implement individualized care plans.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
• Assess and identify resident needs, and communicate this information to the healthcare team
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Monitor resident progress, and adjust care plans as needed
• Maintain accurate and up-to-date records of resident care and treatment
• Participate in quality improvement initiatives to improve patient outcomes and enhance the patient experience
