Setting: hospital-based care; travel temporary assignment

Responsibilities
- Assess patients, review medical history and referrals to determine rehabilitation needs.
- Develop and implement individualized treatment plans to improve mobility, function, and pain management.
- Deliver therapies including therapeutic exercises, manual techniques, gait training, and modalities as appropriate.
- Maintain detailed patient records, track progress, and communicate status to physicians, care teams, and families.
- Coach patients and families on exercises and home programs; provide education to reduce anxiety and promote recovery.
- Collaborate with the rehabilitation team, including speech therapists and other professionals, to optimize outcomes.
- Provide care consistent with patient age and developmental needs; document and participate in care coordination as assigned.
- 90% outpatient, 10% inpatient responsibilities as part of a multidisciplinary team.

Requirements
- Masters Degree in Physical Therapy from an accredited program.
- Current Physical Therapist license (state license required).
- Current Basic Life Support (BLS) certification or ability to obtain within 6 months of hire.
- Experience working directly with patients in a long-term care or hospital setting.
- Ability to work in a hospital setting; strong documentation and verbal communication skills.
- Demonstrated professionalism, teamwork, and commitment to patient-centered care.
