Job Title: Physical Therapist – Home Health

Location: Local community within the service region (home-based patient visits)

Role Summary

Our client, a home health care provider serving patients within their local community, is seeking a compassionate and skilled Physical Therapist to deliver high-quality rehabilitative care in patients’ homes. This role is ideal for a licensed PT who is dedicated to improving patient mobility, independence, and overall quality of life through personalized, evidence-based therapy.

Key Responsibilities

Conduct comprehensive physical assessments and review medical histories to accurately evaluate patient conditions.

Develop, implement, and adjust individualized treatment plans aligned with each patient’s rehabilitation goals.

Deliver therapeutic exercises and functional training programs to improve mobility, strength, and independence.

Educate patients and their families on injury prevention, wellness strategies, and recovery expectations.

Monitor, document, and evaluate patient progress, modifying care plans as clinically appropriate.

Collaborate with a multidisciplinary healthcare team to ensure coordinated and comprehensive patient care.

Maintain accurate and timely clinical documentation in compliance with regulatory and organizational standards.

Required Qualifications

Graduation from an accredited Physical Therapy program.

Master’s degree in Physical Therapy.

Minimum of one (1) year of experience as a Physical Therapist in a home health setting.

Active state licensure to practice as a Physical Therapist.

Current CPR certification (required upon employment).

Valid driver’s license with the ability to travel to patient homes within the assigned service area.

Strong interpersonal skills with a patient-centered, compassionate approach to care.

Experience working independently in a home-based clinical environment.
