Primary Care Physician

Overview ChenMed is a physician‑led, primary care organization that provides full‑risk Medicare Advantage care to seniors. We are expanding rapidly and seek primary care physicians (PCPs) who are willing to lead, coach, and drive outcomes for a panel of up to 450 patients. Responsibilities Provide independent clinical care for older adults with acute and chronic illnesses, including assessment, diagnosis, treatment, management, education, and health promotion. Take full accountability for patient outcomes and seek specialist consultation only when necessary. Engage with hospitalists, care promoters, facilitators, and coordinators, and collaborate with transitional‑care teams for patients in hospital, SNF, LTC, or home. Support and lead the care team, coordinate documentation, and maintain quality of care. Participate in risk and quality‑management programs, clinical meetings, and compliance activities. Cover for other providers when needed and contribute to center management. Recruit patients and additional providers as directed. Perform additional duties as assigned by management. Qualifications Availability and accessibility—willingness to accept walk‑in visits and respond promptly to calls and messages. Service orientation—deliver care as one would for a family member. Evidence‑based medicine—stay current with clinical, nutritional, and lifestyle interventions. Leadership—dedicated to developing and improving leadership skills for the benefit of patients, teams, and the organization. Quality focus—collaborate with teams to achieve high‑quality outcomes and continuously improve practice. Influence—capable of guiding patients and teams toward the best outcomes with compassion. Self‑care—maintain personal health to provide optimal care for others. Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other common office software. Ability to travel locally for training and occasionally regionally or nationally for recruiting. Fluency in English (additional languages may be required in some markets). Pay & Compensation Salary range: $221,141 – $315,915 (base). Compensation will vary by experience, education, geographic location and other factors. Bonuses or commissions may also apply.
