Cardiac Navigator (RN)

Role Overview The position involves assisting patients with complex heart-related concerns and supporting their decision-making throughout their care journey. As a key member of the Cardiology Team, you will contribute to delivering an excellent patient experience by coordinating visits, gathering clinical information, and facilitating additional testing as directed. You will educate patients about their treatment plans, connect them to support services, and promote community outreach to foster patient-centered care. Qualifications & Skills Graduate from an accredited nursing school or Physician Assistant Program Minimum of 2 years nursing experience if RN; alternatively, a master's degree from a PA or NP program Experience working with cardiac disease populations, especially in advanced heart failure, electrophysiology, or structural heart fields Licenses required: Georgia RN licensure and/or multistate licensure, or Physician Assistant licensure in Georgia Key Responsibilities Assist patients and caregivers through the care continuum Coordinate clinical evaluations and testing procedures Educate patients about treatment plans and available support services Engage in community education and outreach activities #J-18808-Ljbffr Azza HealthCare Agency
