Community Health Worker

Peer Support SpecialistEngages and provides peer support to group health members. Serves as a role model in maximizing wellness and independence. Collaborates with clinical and non-clinical team members to ensure a culture of support is reflected in members' treatment.This role supports the Latham/Albany region and their respective surrounding counties across New York State with a Hybrid work model. Applicant must live within the health plan service areas or surrounding counties and will support a hybrid work model with remote work and travel to plan member's residences.Performance Responsibilities:Travels to members' homes to evaluate their plan of care and ongoing needs for community health services and support at least every six months or as directed by Clinical Team.Collaborates with clinical and non-clinical team members to ensure a culture of support in reviewing and updating the members' care plans.Provides support to health plan members, including orientation to the health plan, and the review of members' care plan including services and supportsReports on members' current level of care needs in the community to clinical team members.Refers appropriate questions and issues to clinical and non-clinical team membersConducts outreach to members who are missing monthly contactsAssist with entitlement initiatives such as Medicaid benefits, spend-down, heating and food stamp assistance, and housingCommunicate with internal and external stakeholders in a timely and courteous mannerParticipate in departmental meetings and attend all scheduled staff meetings and staff training as applicable.Perform related duties as required.Requirements:Associates degree in human services, Licensed Practical Nurse, or equivalent of 1-3 years of relevant experience, with high school diploma or equivalency required.Strong computer literacy and comfort, ability, and desire to advance technically and understand industry standards.Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.Valid NYS driving license required with own reliable transportation required.Physical RequirementsAbility to sit or stand at a desk 90% of the dayTravel throughout NYSVisual/hearing ability sufficient to comprehend written or verbal communicationAbility to express self verbally and in writingAbility to use telephone and communicate over the phoneFrequently uses a laptop computer to enter orders and obtain product informationBloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluidsCompensation & BenefitsCompetitive Salary [This position is an S01 exempt position with a min-max rate of: $62,353.20- $69,924.10/annually] with:Retention BonusesInternal Mentoring Program401K with generous Employer matchOn-Site Gym (free for all employees)Potential Hybrid Position (Partial Work From Home)Medical, Dental, Vision plansTuition ReimbursementPartially funded HSAEmployee Recognition PlatformPaid Time Off, Holidays, Sick and Extended Sick LeaveShort/Long term DisabilityEmployee Assistance Program (EAP)Much More!Nascentia Health is leading the way in home care, post acute care and long-term community health.
