Tolstoy Foundation Rehabilitation & Nursing Center is seeking a compassionate, motivated, and resident-focused Occupational Therapist to join our rehabilitation team on a part-time basis. Located in the heart of Rockland County, Tolstoy is a boutique skilled nursing and rehabilitation center with a home-like atmosphere and a strong commitment to helping residents regain independence and return home whenever possible.

Position Summary: The Occupational Therapist evaluates and treats residents with physical, cognitive, and functional limitations to maximize independence and quality of life.

Responsibilities:

• Perform occupational therapy evaluations and develop individualized treatment plans
• Provide skilled occupational therapy interventions
• Assess activities of daily living (ADLs), cognition, safety awareness, and functional mobility
• Develop discharge recommendations and home safety plans
• Collaborate with the interdisciplinary team
• Participate in care plan meetings
• Complete accurate and timely documentation
• Maintain compliance with Medicare, Medicaid, managed care, CMS, and New York State regulations
• Promote resident engagement, independence, and quality of life
• COTA supervision: Provide clinical oversight and guidance to Certified Occupational Therapy Assistants (COTAs) as needed

Qualifications:

• Graduate of an accredited Occupational Therapy program
• Current New York State Occupational Therapist license in good standing
• Skilled nursing facility and/or rehabilitation experience preferred
• Strong clinical assessment and treatment skills
• Excellent communication and teamwork abilities
• Experience with PointClickCare (PCC), Net Health, NetSmart, or similar therapy documentation systems preferred
• Knowledge of PDPM, Medicare regulations, and current post-acute care trends preferred

• Part-time

• Additional hours may be available based on census and facility needs
