Physical Therapist - Day Shift

Location: San Jose, CA

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift with potential weekend coverage required.

Job Description: CVHCare is seeking a qualified Physical Therapist to provide exceptional patient care in the territory covering San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, and Los Gatos. The position involves various responsibilities for patient evaluations, treatment planning, and therapy interventions in a home health setting.

Responsibilities:

• Conduct patient assessments and evaluations.
• Develop individualized treatment plans based on patient needs and goals.
• Implement therapy interventions to improve patient mobility and health outcomes.
• Document patient progress and communicate effectively with healthcare teams.
• Be willing to accept cases including Covid+ and Covid Person Under Investigation patients as required.

Required Experience / Certifications / Licensure:

• Current California Physical Therapist License.
• Basic Life Support (BLS) certification.
• Valid Driver's License and auto insurance.
• Current immunizations including TB, MMR, Varicella, Hepatitis B, TDaP, and annual Flu vaccine.
• Drug screening and physical clearance.
