Licensed Practical Nurse (LPN) - Per Diem

Scope of Position The Licensed Practical Nurse (LPN) provides care and directs the staff to follow the resident plan of care as determined by the Head Nurse. Day in the Life We are committed to providing preventative and restorative care to all residents and clients. The LPN supervises, assigns, and evaluates the work performance of LNA’s on the unit, identifies educational opportunities, performs and signs drug counts at shift change, assesses and reports changes in resident status, and assumes responsibility for admissions, transfers, or discharges. The LPN follows all established safety and infection control policies and procedures, and oversees documentation and reporting in accordance with County policy standards and State/Federal requirements. Required Education And Experience Graduate of an accredited School of Nursing. Must possess and maintain a current, valid, unencumbered New Hampshire LPN license. Three (3) years of Long Term Care (or equivalent) experience preferred; or an equivalent combination of work and experience will be considered. CPR / Basic Life Support (BLS) certified. Knowledge of medical terminology and experience with electronic health records (EHR) systems. Shift Differential 2nd Shift: $4.00/hour 3rd Shift: $5.00/hour Weekend Premium: $2.00/hour Equal Employment Opportunity. Mandatory post‑offer physical and drug testing for new hire.
