Community Health Worker

Description The Community Health Worker supports individuals and families by improving access to healthcare services, increasing health literacy, and connecting clients to essential community resources. This role provides case management, outreach, and advocacy services while promoting overall well-being and independence. The Community Health Worker collaborates with Valley View Health Centers staff, CAC departments, and community partners to deliver coordinated, client-centered care. This position plays a key role in strengthening relationships across healthcare and social service systems while ensuring compliance with program guidelines and confidentiality standards.

- Provide case management services, including:
- Assessment
- Referral
- Monitoring and follow-up
- Assist individuals through:
- In-person visits
- Phone calls
- Virtual communication
- Email correspondence
- Maintain accurate and timely documentation in electronic record systems.
- Ensure compliance with:
- Agency policies and procedures
- HIPAA confidentiality requirements
- Provide:
- Social support
- Informal counseling
- Basic health services such as:
- First aid
- Blood pressure screenings
- Basic wound care
- Demonstrate knowledge of:
- Systems of care
- Benefits and entitlements
- Cultural competency
- Work collaboratively with clients to identify needs and support success.
- Maintain a professional and courteous demeanor with:
- Clients
- Staff
- Funders
- Community partners
- Lead and complete departmental projects as assigned.
- Follow PATHWAYS and Chronic Care Management program guidelines.
- Attend trainings, meetings, and conferences and share relevant information with stakeholders.
- Maintain knowledge of community resources and services.
- Provide referrals to appropriate agencies and programs.
- Serve as a liaison between:
- Valley View Health Centers
- CAC programs
- Community partners
- Ensure compliance with all grant and funding requirements.
- Act as a subject matter expert for Community Health Worker initiatives and partnerships.
- Build and maintain trusting relationships with clients and families.
- Provide:
- Ongoing follow-up
- Goal setting
- Motivational support
- Continuously expand knowledge of available services and programs.
- Represent CAC and Valley View in a positive and professional manner.
- Participate in meetings, trainings, and community engagement opportunities.
- Participate in CAC committee assignments and agency initiatives.
- Support collaboration between Valley View Health Centers and CAC programs.
- Assist with coordination among community partners and providers.
- Perform additional duties as assigned by supervisor.
- High School Diploma or GED required.
- Post-secondary education in a health-related field preferred.
- 1-2 years of experience in a medical or social service setting preferred.
- Valid Ohio Driver's License with:
- Clean driving record
- Reliable transportation
- Insurability under agency policy
- Proficiency in Microsoft Office.
- Ability to obtain Community Health Worker Certification within 6 months.
- Must obtain:
- NPI/Medicaid number upon hire
- QMHS certification within 60 days
- Required screenings and credentialing:
- Background check
- Two-step TB test
- National Practitioner Data Bank (NPDB) query
- Office of Inspector General (OIG) query
- Social work or counseling licensure.
- Training or experience in:
- Mental health
- Substance use disorders
- Knowledge of local community resources and services.
- Experience working in:
- Nonprofit organizations
- Healthcare settings
- Experience with:
- Case management
- Case management software
- Project development and leadership
- Work is primarily performed in a well-lit office environment.
- Fast-paced environment with multiple competing priorities.
- Requires strong organizational and multitasking skills.
- May involve moderate to high levels of stress.
- Occasional evening and weekend travel required.
