RN Care Manager

Location: Roanoke, VA

Assignment Duration: This is a 13-week contract position.

Schedule: Flexible Monday to Friday, from 8:30 AM to 5:00 PM. Consideration for 4/10 schedules available.

Job Description: We are seeking a dedicated RN Care Manager to join our team and bring comfort, compassion, and quality care to patients and families during critical times. You will manage patient care and provide essential services requiring specialized bedside nursing skills for symptom management, crisis intervention, and family education.

Responsibilities:

• Manage patient care and deliver nursing services effectively.
• Provide crisis intervention and support for families.
• Educate patients and families about care plans and symptom management.
• Collaborate with an interdisciplinary team to ensure quality patient care.
• Travel to patient residences, long-term care facilities, and hospitals as needed.

Required Experience / Certifications / Licensure:

• 1+ year experience in hospice care is required.
• Valid driver's license.
• Good driving record and reliable transportation are required.
