This home health LPN traveler role is based in Lombard, Illinois, a vibrant and welcoming suburb located just west of Chicago. In this role, the home health LPN traveler will provide direct nursing care to patients in their homes across Lombard and surrounding western suburbs. Typical responsibilities include performing head-to-toe assessments, managing medications, providing wound care, monitoring vital signs and symptoms, and supporting chronic disease management for adult and geriatric patients. The LPN will deliver patient and family education focused on safety, disease understanding, home treatment plans and prevention of hospital readmissions. A typical day involves traveling to multiple home visits, documenting care and assessments in an EMR system, collaborating with RNs and other members of the interdisciplinary team, and communicating changes in patient status promptly. Visit volumes and daily caseloads are managed to support safe practice and quality care, with scheduling designed to allow adequate time for meaningful patient interaction and thorough documentation. The work environment is field-based, requiring comfort with autonomous practice and strong organizational skills to manage routes, supplies and time. Patient acuity ranges from stable chronic conditions requiring routine monitoring to more complex post-acute cases recently discharged from hospital or rehab settings.
