Medina Meadows – Hotel‑style Skilled Nursing & Rehab Facility

Position Overview
Medina Meadows, a small, hotel‑like skilled nursing and rehabilitation facility, is seeking an experienced and compassionate Nurse to serve as a Unit Manager. The ideal candidate will provide clinical leadership, oversee resident care, and support a team committed to delivering high‑quality, person-centered care in a warm, home‑like environment.

Key Responsibilities

• Supervise and coordinate daily nursing operations on the day shift for the assigned unit.

• Lead, mentor, and support nursing staff — including LPNs, STNAs, and caregivers — ensuring delivery of safe, effective, and compassionate care.

• Conduct resident assessments, ensure accurate documentation, and oversee care plan implementation and updates.

• Collaborate with administration (DON, ADON) to ensure compliance with regulatory and accreditation standards.

• Maintain high standards of infection control, medication management, resident safety, and overall unit cleanliness.

• Facilitate effective communication with residents, families, and interdisciplinary team members.

• Assist with admissions, discharges, and ensuring smooth transitions in care.

• Provide guidance, training, and performance feedback to staff to promote professional development and high retention.

Required Qualifications

• Current, active Nurse license in the state of Ohio.

• Minimum of 1-2 years of experience in long-term care or skilled nursing facility, with at least 1 year in a supervisory or charge nurse role.

• Strong leadership, communication, and organizational skills.

• Demonstrated ability to manage care planning, documentation, and regulatory compliance.

• Commitment to providing care in a respectful, patient‑centered way — treating residents as individuals, not as numbers.

• Ability to problem-solve, multitask, and remain calm during stressful situations.
