Your next nursing opportunity starts here! We are seeking an experienced and compassionate Registered Nurse (RN) with strong OASIS expertise for a full-time home health contract. This 13-week assignment will allow you to build meaningful patient relationships while utilizing your clinical judgment and documentation skills within a structured schedule.
Schedule:
- 13-week contract, full-time
- Monday through Friday
- On-call responsibilities probable
Key Responsibilities:
- Deliver skilled nursing care directly in patients homes
- Complete comprehensive assessments and ensure timely, accurate OASIS documentation
- Coordinate with physicians and care teams to execute individualized care plans
- Perform wound care, administer medications, provide patient teaching, and monitor chronic conditions
- Update clinical teams promptly regarding changes in patient status
Preferred Qualifications:
- Active, unrestricted RN license
- At least 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase (highly preferred)
- Ability to manage a full-time caseload and structured daily visits
- Reliable transportation and valid drivers license
