RN Case Manager | Home Visits, Grand Rapids Area Join Our Team as an RN Case Manager for Home Visits in the Grand Rapids Area. Atrio Home Care, the partnership of Holland Home, Brio and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Team. This position will involve providing direct care to clients within a 35-mile radius of Grand Rapids. $111,000.00 yearly Full-time Responsibilities Perform initial and comprehensive assessments and deliver compassionate care to clients Collaborate with physicians and other care team members to develop a Plan of Care Promotes client and family/caregiver independence through counseling and education Identify barriers to desired outcomes and evaluates outcomes of care Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system Participates in on-call rotation Location Grand Rapids, covering a 35-mile radius around GR Schedule/Shift Monday - Friday Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance Compensation Our compensation model includes bonuses for high-quality client outcomes and efficient case management Premium pay for weekend & holiday visits Qualifications Licensed RN in Michigan; BSN preferred Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred Passion for delivering high quality care to older adults Paid Time Off (PTO) Generous 403b match with access to a professional Financial Planner at no cost Why work for Atrio?
