Providence Center Residential Case Manager Atwood Street
The Providence Center Residential Case Manager Atwood Street will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.
Duties And Responsibilities
- Coordinate and deliver person-centered, recovery-oriented case management as part of a multidisciplinary team.
- Work closely with the care team to align case management services with clients' mental health treatment plans.
- Conduct comprehensive assessments to identify client needs related to housing, education, employment, financial stability, healthcare, and community resources.
- Develop, present and implement individualized care plans in collaboration with the care team, clients and their families as appropriate.
- Provide support counseling with mental health issues, substance use disorders, and medical conditions.
- Provide ongoing monitoring and follow-up to ensure client needs are met and progress is achieved. Implement emergency protocols when required.
- Assist clients with daily activities and life skills through modeling and providing education to promote independence. This includes medication management and budgeting as appropriate.
- Supervise clients self-administering medications while using the 6 Rights of Medication Self-Administration.
- Assist and monitor the group home clients with medical interventions and hygiene needs.
- Conduct daily room/unit inspections.
- Provide and maintain a home atmosphere.
- Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities.
- Negotiate and schedule appointments for clients within The Center and with community partners (Social Security Supplemental Insurance (S.S.I.), SNAP, Advocates Office, Medical Doctor, etc.).
- Connect clients to community resources, including social services, transportation, and financial assistance programs.
- Serve as a liaison between clients and community agencies, advocating for client needs and facilitating access to services.
- Maintain and promote family contacts and relationships.
- Provide education and guidance to clients and families on navigating systems such as healthcare, education, and social services.
- Identify, assess and respond to clinical emergencies and crisis situations, including suicide risk and danger to others. Perform crisis intervention and risk management as needed, escalating to appropriate resources when indicated (Qualified Mental Health Professionals/QMHPs or Emergency Services).
- Maintain accurate, detailed, and timely records of all client interactions and services provided, including clinical documentation as required.
- Prepare reports and documentation as required for audits and program evaluation.
- Ensure compliance with agency policies, ethical standards, and regulatory requirements.
- Attend team meetings and case reviews to share updates and collaboratively address client challenges.
- Develop professional relationships with Day Program staff.
- Foster cooperative relationships with staff, clients, and community partners.
- Transport clients safely to appointments and activities as needed.
- Enter Work Request Orders for facility-related issues as needed and communicate such to department team.
- Attend trainings and case presentations.
- Serve on committees and participate in agency initiatives as assigned.
- Perform other duties as assigned to support client goals and program needs.
Requirements
- High School or GED Required
- Minimum 1 to 3 Years of experience in a behavioral health setting. Experience in residential facilities preferred.
- Must have valid driver's license, registration, current inspection and insured automobile.
- Manual dexterity and mobility required. While performing the major responsibilities of this position, the incumbent must be able to regularly sit, stand or walk, and occasionally reach for prolonged periods of time. Must be able to lift, push, move and/or carry supplies or equipment up to 25 pounds, and occasionally be able to bend, stoop, crouch or kneel.
- Residential setting for adults with severe psychiatric illness that require 24/7 monitoring.
- Strong knowledge of community resources and systems, including housing, education, employment, and healthcare.
- Ability to build rapport and establish trust with diverse clients and community partners.
- Cultural competence and commitment to providing equitable and inclusive services.
- Ability to communicate effectively, both verbally and in writing, to convey information clearly and collaborate with multidisciplinary teams.
- Excellent organizational, problem-solving, and time management skills.
- Proficiency in documentation and electronic record-keeping systems, including ability to document in electronic medical records.
- Ability to travel within the community with reliable transportation.
- Able to work flexible hours, including evenings, weekends, holidays and during inclement weather, as well as cover the program when unforeseen circumstances arise to ensure 24-hour coverage.
- Bilingual preferred. For positions designated as bilingual (Bil), proficiency in the required language is mandatory, and the language assessment must be passed no later than 30 days of starting the role.
