Family Practice - Without OB Physician

• Provide comprehensive primary care services to patients and families throughout the lifespan.
• Manage preventive health, acute conditions, chronic diseases, behavioral health needs, and community health concerns.
• Serve as the primary care provider and coordinate appropriate medical services with internal and external specialists.
• Support high-risk patients through care management programs designed to reduce hospital and emergency department utilization and improve health outcomes.
• Maintain accurate patient records, care plans, and care management documentation in the electronic health record.
• Ensure communication with patients is culturally and linguistically appropriate.
• Participate actively in clinic meetings, patient-centered medical home (PCMH) activities, team huddles, quality initiatives, and other organizational programs.
