Nurse Practitioner

We are seeking an experienced and patient-focused Nurse Practitioner (NP) to provide comprehensive primary care services within a Family/Internal Medicine clinic. The Nurse Practitioner will work collaboratively with physicians and medical assistants, and support staff to deliver high-quality care to patients across all age groups.

This role offers the opportunity to manage a diverse patient population while practicing independently within a supportive clinical environment utilizing Epic EMR.

Location: Rialto, California

Position type: Minimum 6-month temp/contract role (possible extension)

Schedule: Fully On-Site | Monday - Friday | 8:00 AM - 4:30 PM

Pay Rate: $ 72.37/hour


Key Responsibilities

• Perform comprehensive patient examinations and health assessments


• Obtain and review detailed medical histories


• Diagnose and manage acute and chronic medical conditions


• Order, perform, and interpret diagnostic and laboratory tests


• Prescribe medications and treatment plans in accordance with state regulations and clinical guidelines


• Conduct Annual Wellness Visits and preventive health screenings


• Complete post-hospital discharge follow-up visits and transitional care management


• Manage patient communications and care coordination through Epic In-Basket messaging


• Provide patient education regarding diagnoses, treatment plans, medications, and preventive care


• Collaborate with physicians, medical assistants, and interdisciplinary team members to ensure continuity of care


• Maintain accurate and timely clinical documentation in Epic EMR


• Adhere to all organizational, regulatory, and clinical quality standards


Patient Population

• All ages


• Average daily patient volume: 18-22 patients


Support Staff

• Dedicated 1:1 Medical Assistant support


Required Qualifications

• Graduate of an accredited Nurse Practitioner program


• Current California Nurse Practitioner License


• Board Eligible or Board Certified Nurse Practitioner


• Current BLS/CPR Certification


• Active DEA Registration


• Ability to independently evaluate, diagnose, and manage patients in an outpatient setting


Preferred Qualifications

• Minimum of one year of outpatient clinic experience


• Family Practice or Internal Medicine experience


• Experience utilizing Epic EMR


• Strong communication and patient education skills


• Ability to work efficiently in a fast-paced clinical environment
