Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment aimed at healing and wellness. Nursing practices are based on evidence, current research, hospital policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process along with critical thinking and creativity. Nursing excellence is promoted through helping relationships among nursing staff and other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes provision of nursing rounds, assessments, and treatments. The RN will facilitate admission, transfer, and discharge activities. Provision of patient and family-centered education is a key responsibility. Care coordination and individualized treatment planning will be carried out as applicable. The RN assesses patient needs and performs comprehensive assessments in accordance with scope of practice and hospital policies. Consideration of patient culture, age, and developmental level is essential. The RN identifies patient risk factors and presenting complaints. Patient health information may be obtained from family or others as necessary. Objective and subjective data is collected. The initial admission assessment will be verified if obtained by other healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in coordination with the patient and family according to the medical plan of care. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to the patient and family. Action is initiated to alleviate patient stress and discomfort. Changes in patient conditions are recognized, responded to, and communicated appropriately. Ongoing assessment and responses to procedures and therapies are conducted and documented using the nursing process. Tasks are delegated appropriately, overseeing team members in the performance of activities regarding patient care. The RN monitors the accuracy and completeness of delegated patient care provided by other team members. Educational needs of patients related to prioritized problems are identified and addressed. The discharge process is facilitated by executing discharge orders in a timely manner. Potential discharges are identified, and discharge teaching is performed, coordinating with case management and others as indicated. Input into patient/nurse assignments is provided considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. A positive group dynamic and healing environment is promoted through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practices, and current research. Participation in quality improvement activities is expected, focusing on patient satisfaction, falls, pressure ulcers, and documentation standards. A holistic perspective regarding patient care is utilized at all times.
