Care Navigator (Licensed Clinical Social Worker)

Wovenly integrates remote dementia-focused care navigation with local health care providers, providing resources and services supporting families and patients living with dementia. About the role: As Wovenly’s Care Navigator, you will be a guide to both caregiver and patient with dementia. You’ll work virtually across health care settings and community organizations, ensuring our members are connected to appropriate resources, are following the care plan created by their providers, and are ensuring needs are met while providing a positive experience for the member. You will need to have a service-oriented mindset, be passionate about senior care, and be eager to learn and grow with a new company. This is a critical early team role, and in it you’ll report into our Co-Founder and CMO. What you’ll do: Get to know the Wovenly model, patient populations, and community resources needed to guide care in the best possible way for them (this could be over video, email, the phone, or text messages). You’ll be key to help assess for unmet SDOH needs, take a comprehensive health history, and create plans to address gaps in care; including Advance Care and End of Life Planning. Enroll members into the Wovenly program. You’ll help them understand the benefits of the program (at no cost to them). You’ll also get to know their care plan and you’ll lead the first needs-assessment for the member so we know how to best support them throughout the dementia journey. You’ll schedule appointments for the patient and their caregiver with community services, their healthcare providers, or other specialists as needed. You’ll also guide referrals and make sure the right follow-up actions are taken. If members need support connecting with providers or resources (e.g., transportation, financial grants, emotional support resources, medical equipment) you are responsible to get it for them. Build the foundation for Wovenly’s care support program so other Care Navigators can follow in your footsteps. This includes developing educational materials, supporting the build of our resource database, and enabling care protocols. Everything you build and do maintains the highest standards of privacy and HIPAA compliance. What we’ll expect from you: By the end of your first month you will have: Welcomed new patients and their caregivers to Wovenly and introduced them to care navigation, the resources, education, and support that you via Wovenly can provide them. You’ll manage Wovenly’s phone line and be the first point of contact for any inbound call or email. You may have to take messages for team members, triage patient calls, or connect with providers - and you’ll need to be flexible to adapt to the requirements of the moment. Built initial tools that you can use to provide education and support to new patients and caregivers around dementia. You’ve also helped to develop, review, and update Wovenly’s community resource database and care protocols that relate to abuse, neglect in dementia care. Developed the ability to screen for unmet care needs, including clinical or medication issues, behavioral issues, safety risks, and psychosocial well-being. You’ll use assessment tools and templates to support this, and you may improve them where you see need. Become a bridge to local community services and public benefits. You’re tracking these resources and connections in our database and updating when needed. Be comfortable using and documenting all interactions with patients and caregivers in our EHR. Data is correct and By the end of your first three months you will have: Built trusting relationships with your patients and their caregivers, where active listening and empathy are the foundation for your interactions with them. You use that relationship to prioritize how we continually address their needs. With the strong relationships you’ve built, you’ll stay close to the patient’s caregiver to learn about side effects and possible changes in the patient’s function or behavior. You’ll use this insight to support Providers with medication management strategies. Comfort consulting with licensed clinical team members (i.e. the patient’s PCP) and triage complex or medical issues to them. You will participate in and contribute to patient rounds with the interdisciplinary care team weekly. What leads to success: We believe you’ll thrive in this role if the following describes you: Experience & Qualifications . You have a Bachelor’s degree and Master of Social Work (MSW) or MSW-equivalent program and have an active Licensed Clinical Social Work (LCSW) license. You’ll need to have at least two years of clinical work experience in case management or care coordination, and ideally have worked with senior populations or patients living with dementia. It’s critical you have a working knowledge of medical terminology. Preferred to have an active California License. Comfort with Technology . You’re comfortable using Slack, Zoom, Google Suite, and EHR systems. Strong relational skills . You have an uncanny ability to establish rapport and work effectively with diverse populations including health care providers, clinical staff, patients, and family members. Bias to action . You don’t wait for someone to tell you what to do. You know how to identify and prioritize your initiative’s needs, and do what it takes to ensure that urgent and important needs are acted on immediately. Comfort with change and ambiguity . You may not always have direction in your work, and that’s OK, because you’re comfortable figuring out what needs to be done and a path forward. Organized & Communicative . You are incredibly organized and sweat the details. You keep your team in the loop with what you’re working on and the challenges you’re facing. Professional .
