Responsibilities
- Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs; document in the care plan.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors and incorporate into the POC.
- Maintain up-to-date patient records, ensuring visits, contacts, problems, plans, actions, and goals are accurately reflected and updated.
- Provide care utilizing infection control practices and comply with OSHA standards; educate patients and families on care, medications, home safety, and related topics.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.

Requirements
- Current state license as a Registered Nurse (Active Maine RN license required).
- 1-2 years of RN clinical care experience; 2+ years of home health experience required.
- Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program; Bachelor of Science in Nursing preferred.
- CPR certification required; CHPN certification preferred.
- Proficiency with Homecare Homebase software and OASIS experience required.
- Valid driver’s license with reliable transportation; ability to travel locally to patient homes.
- Ability to work independently, manage multiple priorities, and communicate effectively with patients, families, and the care team.
