You will lead decisions on GDMT for heart failure and guidelines for T2D and hypertension, with asynchronous titration and close collaboration with physicians and care teams. The role is fully remote, Pacific Time-aligned, with strong emphasis on productivity, quality, and collaboration across RNs and care teams.
