Field-Based Case Manager - Family Care Partnership

Humana/iCare in Milwaukee County is seeking a Case Manager to provide psychosocial assessments, care planning, and ongoing care coordination for adults with disabilities and the frail elderly. You will work with the Interdisciplinary Team to identify needs, access services, and monitor outcomes while upholding HIPAA and care standards.
