Our GREAT MANAGERS believe YOU matter, and YOUR voice is heard.Job Purpose:Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN/RN will report directly to the Director of Nursing.Skills/Qualifications:LPN must be 21 years of age with a HS diploma and a Certificate of Practical Nursing and/or Associate Degree, and have a current unrestricted license to work in the state of Missouri and be CPR certified. The LPN/RN must have a minimum of 2 years of direct patient contact with either a Skilled Nursing Facility or an In- Home Care Agency.Job Responsibilities:Provide skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs, and preventive care.Utilize a skilled approach in the provision of problem-specific therapeutic interventions, teachings, and training activities in accordance with the plan of care.Assists with admissions and assessmentsRegularly visits LTC and Hospital facilities to assist with outreach and marketingDevelops sales by making initial presentations; explaining products and services; and by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, and community stakeholders on Home Care Services and the admission process.Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; and supervising assigned team members.Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.Provide hands-on care and patient education utilizing At Home Care Policy.Treat wounds, change wound dressings, give injections, administer enemasCollect specimens from patientsAdministers medications and sets up medication dispensers.Measures and records vital signsReports and observes changes in patient conditions to the Director of Nursing
