Submission Requirements:

Qualifications:

• Minimum of 1 year of inpatient Physical Therapy (PT) experience - Required
• Orthopedic (Ortho) experience - Highly Preferred
• Previous travel experience - Highly Preferred
• Graduation from an accredited Physical Therapy Assistant (PTA) institution - Required
• Michigan (MI) PT License - Required

Profile Standards:

• Compliance with health system-wide profile standards

Skills Checklist:

• A skills checklist completed within one year of application for PT specialties is required.

Certification Requirements:

• Basic Life Support (BLS) certification from the American Heart Association (AHA) - Required

State License Requirements:

• Michigan (MI) State License - Required
• Michigan license verification is required for PTs. Please provide the necessary documentation, including LARA and Nursys. In case of any license issues, provide a written explanation for the manager's review.

Certification Verification:
