Board: Board Certified
Licenses: Active ME License or Universal License (IMLC)
Experience: Locum experience required; overall years of experience required (specifics to be noted at presentation)
Other Qualifications: EPIC experience required; NPDB report within 30 days; 2 supervisor and 1 peer references from 12 months; internally credentialed with agency; clean professional history (no licensure/employment actions)

Duties & Responsibilities

Provide acute and chronic care management for adult patients
Conduct annual wellness visits and post-hospitalization follow-up appointments
Manage in-basket tasks including prescription refills and patient advice requests
Follow up on diagnostic test results and coordinate specialist referrals
Collaborate with a multidisciplinary team to ensure high-quality patient care
