Job Type Part-time Schedule Monday - Friday, 8:30am-5pm Position Summary The LPN is responsible for assessing the medical needs of potential and current clients, ensuring that services provided are appropriate and meet their healthcare requirements. Responsibilities Develop and update comprehensive care plans. Provide staff training, ensure accurate documentation, and support medication management. Conduct in-home health assessments and address medical concerns. Lead caregiver training and evaluate competency in essential medical tasks. Maintain accurate health records and electronic documentation. Communicate effectively with families, caregivers, and team members regarding client needs and updates. Work under RN supervision while operating independently in the field. Requirements Licensure: Valid State LPN License. Compact license preferred. Education: High school diploma or GED required. Experience: Minimum 1 year in home care, healthcare, or senior-related industry (preferred). Technical Skills: Proficient with software tools such as Adobe, Outlook, Teams, Word; ability to learn new EMR systems. Communication: Excellent written and verbal communication skills. Training Ability: Comfortable leading caregiver training sessions for groups of 5–10. Transportation: Reliable and consistent transportation for travel within assigned territory.
