This role provides education on nutritional status, nutrition care planning, physical activity, disease process, diabetes medication, problem-solving, coping, and risk reduction. The Dietitian also informs patients about the important factors associated with the disease and the preventive measures they need to consider while treating it. Supports patients in improving and maintaining their overall health. Provides patient, staff, and community education.In our time of Company growth, we are seeking a Dietitian.Work Location: Bradenton, FLAs part of this role, you will:Promote the mission, vision, and values of the organizationProvide outpatient diabetes self-management training and education to type 1, type 2, and gestational diabetes patientsPlan, develop, and implement individual and group diabetes education for adults, their families, and caregiversInstruct patients in the Standards of CareProvide education related to diabetes and the disease process; medication, diet, and nutritional monitoring, foot care prevention, and the techniques of self-monitoring and injectionAdvise patients as to the potential for long-term complicationsProvide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as requiredParticipates with the medical team, assuring diabetes clinical care and management are consistent with ADA standards of care and guidelinesDocument assessments, plans, and goals in the Electronic Medical RecordIdentify opportunities to improve patient/customer experience.
