Transitions At Home , is currently seeking to hire a Full-Time compassionate and dedicated Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients in the Baraboo area. What does a Transitions at Home LPN do? Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
