Our dedicated, hardworking staff services all of Arizona, parts of Nevada and California, New Mexico and Texas. Our team works with our patients to establish a trusted relationship for care continuum.

Description:

A Registered Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence.  This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

Your case load is entirely up to you. Whether you are a workaholic who wants to see 5 patients a day or 2-3 a week, across your first month you set the pace in which we assign work to you. Never do we set a time for you to see the patient, but you get to call and arrange your visits to what works for both you and the patient. Typically visits are 60 minutes.

Qualifications:

• Active OT License in the state of practice
• Valid Driver License
• Valid Auto Insurance
• AZ Level One Fingerprint Card 
• CPR or BLS Certification
• Valid TB Test (Skin test within the last year or x-ray within the last five years)

Job Type: Pier - Diem

Work Location: On the road 

West Valley : Peoria, Sun City, Surprise, Glendale, Goodyear, Buckeye etc.

East Valley : Mesa, Chandler, Gilbert, Tempe etc.
