Physical Therapist (PT) - Acute Therapy

Role Overview: Evaluates and treats patients of all ages, developing and adjusting personalized care plans to achieve optimal outcomes. Acts as a clinical resource and role model for staff and students, ensuring delivery of professional services with respect and empathy. Key Responsibilities Designs, implements, and modifies treatment plans based on patient response and evidence. Educates patients, families, caregivers, and healthcare providers using effective teaching methods. Performs examinations, clinical evaluations, and screens to determine diagnoses and therapy needs. Documents clinical findings and outcomes in compliance with organizational policies. Collaborates with the healthcare team, communicates patient progress, and provides discharge and equipment recommendations. Supervises support personnel, monitors care quality, and provides feedback. Utilizes standardized outcome measures to assess functional performance. Provides family training and adjusts interventions considering patient behavior and emotional needs. Qualifications & Skills Bachelor's Degree in Physical Therapy. Licensed Physical Therapist (PT) in Illinois. Basic Life Support (BLS) certification within 30 days. Experience with acute stroke patient care and knowledge of body mechanics and safe transfer techniques. Strong communication, documentation, and patient education skills. Ability to work collaboratively within a multidisciplinary team and supervise support staff.
