About This Role The Staff Nurse RN in Primary Care provides nursing care to a defined group of patients, coordinating and facilitating health care services in collaboration with the health care team. This role is ideal for nurses who are dedicated to patient assessment and care plan implementation. Highlights Pay: $51 - $93/hour Location: Seattle, WA Facility: Kaiser Permanente Schedule: Full Time What You'll Do Provide ongoing assessment of nursing care needs for a defined group of patients. Develop a written plan of care based on nursing diagnosis and patient needs. Assist patients and families in developing health care goals. Provide nursing interventions safely and competently. Offer telephone consultation to patients and families. Evaluate patients' responses to care and modify care plans as needed. Assess the needs of patients requiring hospitalization and initiate discharge planning. Communicate patients' psychosocial, cultural, and emotional needs during interactions. Develop educational components for nursing care plans and provide teaching/counseling. Promote patient self-care responsibility and adhere to safety and infection control standards.
