Job Title: Physical Therapist Location: Ashland, AL Job Description: Performs physical therapy evaluation and treatment based on physician orders/referrals. Completes comprehensive evaluations using appropriate assessment tools. Establishes measurable goals and develops treatment plans for patient independence. Regularly re‑assesses treatment plans and modifies goals as needed. Communicates patient needs and progress to the treatment team and family members. Consults with medical providers regarding patient progress and treatment. Maintains and submits documentation in compliance with regulations and standards. Shift Requirements: 8 am – 4:30 pm, on‑site work location, 40 hours guaranteed. Education / Licenses: Doctor of Physical Therapy (DPT) required.
